Requirements for Application
- Proceed to the Office of the Registrar for Application Requirements;
- Pay the non-refundable College Entrance Examination and Processing Fees at the Hospital’s Cashier located at theGround Floor, Hospital Bldg.;

- Present Official Receipt to the Registrar and secure Application Form and list of requirements;
- Fill-out Application Form and attach the required documents (Only Application Form with correct and complete information/ requirements shall be processed);
List of Requirements :
- For Freshmen
a) Certification from the Principal/Director/Registrar/Guidance Counselor that the student belongs to the graduating class (original and 2 photo-copies)
b) NSO Birth Certificate (original and 2 photo-copies)
c) Three (3) pieces 2X2 pictures
d) One Long Brown Envelope
- For Trasferees
a) Transcript of Records for Evaluation
b) Transfer Credential Certification/Honorable Dismissal Certification
c) NSO Birth Certificate (original and 2 photo-copies)
d) Good Moral Certification from the Guidance Counselor
e) Three (3) pieces 2X2 pictures
f) One Long Brown Envelope

- Submit accomplished Application Form and other requirements at the Registrar’s Office and secure Examination Permit for College Entrance Test;
- Take the College Entrance Test as scheduled on the Examination Permit (No re- scheduling of Entrance Examination is allowed)

INFORMATION SHEET
FOR NON-RESIDENT FILIPINO AND NON-RESIDENT ALIEN SEEKING
ADMISSION TO THE COLLEGE OF PHYSICAL THERAPY
FOREIGN STUDENT seeking admission to the UERMMMC College of Physical Therapy while abroad, applies in writing directly to the Dean, College of Physical Theraphy or to the Registrar, UERMMMC, 62 Aurora Blvd., Quezon City 1113, Philippines and must submit the following documentary requirements:
- Five (5) copies of the High School Transcript of Records duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence, not “NOTED & SEEN” but with C-167-OCA-99 with red ribbon and seal (1 original and 4 photo-copies);
- High School Diploma or certificate of graduation duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence, not “NOTED & SEEN” but with C-167-OCA-99 with red ribbon and seal (1 original and 4 photo-copies);
- Seven (7) copies of Original Personal History Statement form (PHS, Revised 1998), duly accomplished and signed by the applicant in English and in national alphabet, accompanied by personal seal, if any, original left and right hand prints and original photos;
- A notarized Affidavit of Support and Proof of Adequate Financial Support, i.e., bank statement, to cover expenses for student’s accommodation and subsistence, as well as school dues and other incidental expenses; (1 original and 3 photo-copies)
- Original and 3 photo-copies of Birth Certificate
- Original and 3 photo-copies of Passport
- Original and 3 photo-copies of Police Clearance duly authenticated by the Philippine Embassy or Consulate in the applicant’s country of origin or legal residence, not “NOTED & SEEN” but with C-167-OCA-99 with red ribbon and seal;
- Certificate of Good Moral Character signed by the principal or two (2) instructors from the High School and
- Application Fee: ------ $100.00 (U.S.D.)
- Miscellaneous Foreign Fee: ----- $1,000.00 (U.S.D.) Payable upon Enrollment.
Admission Procedure
- UERMMMC, College of Physical Therapy receives accomplished application form together with the other documents required for a prospective applicant;
- List of successful examinees will be posted on the Bulletin Board of Registrar’s Office, Dean’s Office, College of Physical Therapy and UERMMMC’s website: www.uerm.edu.ph;
- Accepted Applicants and their Schedule Date of Enrolment will be posted on the Bulletin Board of Registrar’s Office, Dean’s Office, College of Physical Therapy and UERMMMC’s website: www.uerm.edu.ph;
Enrollment Procedure
Step 1
1.1 NEW STUDENTS (FRESHMEN)
- Proceed to the Registrar’s Office and submit the following entrance credentials for Admission
- Original and two (2) photo-copies of High School Card (F-138) with remarks “GRADUATED”/”PROMOTED TO COLLEGE”
- For Valedictorian or Salutatorian, certification from the High School Principal/Registrar indicating the honor or academic award obtained and the number of graduates
- One Long Folder
- Two (2) Self-Stamped Envelope
- Secure to the Registrar’s Office and fill–out the following forms:
- Advising Slip (3 copies)
- New Student Information Sheet (2 copies)
- Proceed to MISD (2ND Floor, Admin. Bldg.) for encoding and assessment.
- After assessment, check the details of your registration and sign on the space provided on your registration form. (Please proceed to … Step 2)
1.2 NEW STUDENTS (TRANSFEREES)
- Proceed to Dean’s Office, College of Physical Therapy – 4th Floor, Administration Building
- Secure an Advising Form and the New Student Information Sheet. Have these forms accomplish.
- List properly your subjects and section to enroll as advised.
- Proceed to MISD (2ND Floor, Admin. Bldg.) for encoding and assessment.
- After assessment, check the details of your registration and sign on the space provided
- on your registration form. (Please proceed to … Step 2)
Step 2
- Proceed to the Cashier (across the street Hospital Lobby) for payment.
- Present your 3 copies of registration form together with your payment.
- Secure your printed O.R. and the student copy of your registration form.
- (Two other copies, Accounting & Registrar will be left at the Cashier)
- Pay an ID FEE
- DO NOT LOSE YOUR O.R.
Step 3
- Proceed to the Infirmary Section of the Hospital for schedule of Physical Check-up.
Step 4
- Proceed to MISD (2nd Floor, Admin. Bldg.) for the schedule of ID picture taking;
- Present your O. R. ID Fee and ID Payment slip
- Have your picture taken and wait for its issuance.
NOTE: COMPLETE UNIFORM IS REQUIRED BEFORE PICTURE TAKING.
