    1. Minimum Qualifications for Admission

Applicants seeking admission as a freshman to the UERM Doctor of Medicine degree program must have the following qualifications:

 

      1. Holder of a baccalaureate degreeApplicants who have not yet graduated must be able to submit a certificate of candidacy for graduation issued by his/her higher educational institution.He/She must have graduated on or before the start of the regular enrollment period of the UERM College of Medicine.
      2. Must have taken the National Medical Admission Test (NMAT) not more than two (2) years from the time of admission with a percentile score of 70 or betterTo be admitted as a freshman for S.Y. 2025-2026, the valid NMAT scores are those taken on the following dates:

        2023 January, April, May & October
        2024 January, February, May & October
        2025 January & February

         

        The application of the following applicants will be processed only upon signing, and complying with, the Undertaking for NMAT: (NMAT) Form

         

        • Current NMAT percentile score does not meet the cut-off of the UERM College of Medicine
        • Yet to take the NMAT

        4. Filipino citizen who obtained his/her baccalaureate degree from a higher educational institution outside the Philippines

    1. Application Requirements
      1. For Both Local and Foreign Applicants
        1. One (1) original copy of birth certificate
        2. One (1) original copy of official transcript of records issued by the applicant’s higher educational institution
        3. One (1) original copy of NMAT percentile score
        4. One (1) original copy of certificate of good moral character issued by the applicant’s higher educational institution
        5. Recommendation letter from two (2) of the applicant’s professors in college
        6. One (1) certified true copy of diploma or one (1) original certificate of candidacy for graduation issued by the applicant’s higher educational institution, whichever is applicable
        7. Two (2) printed copies of signed online application form
        8. Upload 2X2 picture in White background
        9. One (1) printed copy of accomplished Undertaking for NMAT, if applicable (NMAT) Form
        10. Vaccination Card (PHOTOCOPY)

         

      2. Additional Requirements for Foreign Applicants Only
        1. One (1) original copy and two (2) photocopies of diploma or certificate of graduation with “Apostille”
        2. One (1) original copy and two (2) photocopies of official transcript of records with “Apostille”
        3. Six (6) original copies of accomplished Personal History Statement Form (Revised 1998) PERSONAL HISTORY STATEMENT Form
        4. One (1) original copy and two (2) photocopies of notarized affidavit of support and proof of adequate financial support (i.e., bank statement to cover the expenses for the student’s accommodation and subsistence as well as school dues and other incidental expenses)
        5. One (1) original copy and two (2) photocopies of the notarized police clearance in the applicant’s country of origin or legal residence
        6. Photocopy of the pages of the valid passport/s which shows the name, picture, birth date, and birth place of the applicant
        7. One (1) original copy and two (2) photocopies of clearance from the National Bureau of Investigation (NBI) (For foreign applicant who visited the Philippines at least once)
        8. Eight (8) pieces of 2 x 2 ID picture with name tag (in white background)

         

       

Accepted applicants are required to submit all original documents not later than August 2, 2024

  1. Online Application Procedures
    1. Ensure that your e-mail address (preferably GMail) and mobile number are active and working.
    2. Accomplish the application form online through this link: Application Form.
    3. Submit through courier the printed copy of the signed online application form together with the documents contained in a long plain folder and long brown envelope.A dropbox will also be made available in the emergency exit gate of the North Campus.
    4. Upon receipt of your documents, the Office of the Admissions will send an acknowledgment receipt, through e-mail, of the checklist of documents we received from you.
    5. Instructions on how to pay the non-refundable application fee will also be sent through e-mail. The non-refundable application fees for admission as freshman to the UERM College of Medicine are as follows:
      • Local applicants – ₱3,000
      • Foreign applicants – US$300

       

    6. Upload your proof of payment through this link: UERM UPLOAD PROOF OF PAYMENT form
    7. After validation of your payment, you will receive an e-mail and text message on how to select the date and time schedule of your online interview.
    8. The successful applicants will be informed in the UERM Applicants’ Portal. They will also be informed through text message (for local applicants) or e-mail (for foreign applicants).

Graduate School

  • Master of Arts in Nursing
  • Master of Science in Nursing
  • Master of Health Care Management
  • Master of Health Science Education
  • Master of Science in Asian Health Practices

  • Master of Science in Tropical Medicine

 

    1. Basic Requirement:

      • Graduate of four (4)-year baccalaureate degree program

      • For applicants of the Master of Science (MS) / Master of Arts (MA) in Nursing degree program – Must be a holder of a Bachelor of Science degree in Nursing

    2. Application Requirements [Original copy with two (2) photocopies placed in a long white folder]

      1. Official transcript of records

      2. Transfer credential certificate

      3. For applicants of MS / MA Nursing: License or certificate from the Professional Regulation Commission (PRC)

      4. Certificate of professional training

      5. Two (2) recommendation letters from former professors, colleagues or supervisors

      6. Diploma (certified by the University Registrar)

      7. Birth certificate authenticated by the Philippine Statistics Authority (PSA)

      8. Marriage certificate authenticated by PSA, if applicable

      9. Three (3) passport size colored picture with white background duly signed at the back

      10. Application letter addressed to:

Additional requirements for foreign applicant:

      1. Official transcript of records with “APOSTILLE”

      2. Diploma with “APOSTILLE”

      3. Passport pages which shows the name, picture, birth date and birth place of the applicant

      4. For foreign applicants who visited the Philippines at least once: One (1) original copy and two (2) photocopies of clearance from the National Bureau of Investigation (NBI)

      5. One (1) original and two (2) photocopies of notarized police clearance in the applicant’s country of origin or legal residence

      6. If English is not the medium of instructions in the country of origin of the applicant: TOEFL result of at least 500 or equivalent

      7. Proof of license to practice in the country of origin of the applicant

Applicants seeking admission as a freshman to any of the UERM undergraduate degree programs must be a holder of a senior high school diploma.

He/She must have graduated on or before the start of the regular UERM enrollment period.

      1. Local Applicants

        The following are considered as local applicants:

        1. Resident Filipino citizen
        2. Filipino with dual citizenship [with recognition and proof of residency of at least five (5) consecutive years in the Philippines]

         

      2. Foreign Applicant

        The following are considered as foreign applicants:

        1. Resident alien
        2. Non-resident alien
        3. Non-resident Filipino (immigrant of another country)
        4. Filipino citizen who obtained his/her baccalaureate degree from a higher educational institution outside the Philippines

      1. Certificate of Live Birth issued by the Philippine Statistics Authority
      2. Grade 11 Report Card or Certificate of Secondary Scholastic Record (CERTIFICATE OF SECONDARY SCHOLASTIC RECORD) signed by the principal or registrar, whichever is available.
      3. 2 x 2 ID picture in white background

       

    2. Additional Requirements for Foreign Applicants Only

      Foreign applicants should e-mail the scanned copy of the following documents to the UERM Admissions Office (admission@uerm.edu.ph):

       

       

      1. Diploma or certificate of graduation with “Apostille”
      2. Official transcript of records or complete copy of grades with “Apostille”
      3. Accomplished Personal History Statement Form PERSONAL HISTORY STATEMENT Form (Revised 1998)
      4. Notarized police clearance in the applicant’s country of origin or legal residence

      6. For resident aliens: Clearance from the National Bureau of Investigation (NBI)

  2. Online Application Procedures

     

    1. Ensure that your e-mail address and mobile number are active and working.
    2. Accomplish the online application form in the UERM Student Admission Portal (https://uerm.edu.ph/apps/admission) and upload the scanned copy of the basic document requirements. You may also access the portal by scanning the QR code provided.
    3. You will receive an automated acknowledgement receipt via e-mail after validation of your uploaded documents by the UERM Admissions Office. Please check your UERM Student Admission Portal to select your preferred interview schedule.
    4. After the interview, successful applicants will be notified through e-mail of their enrollment schedule and requirements.

    6. Deadline of application for S.Y. 2024-2025 is on July 12, 2024

     

  3. Enrollment Procedures

    Successful applicants should submit the following original documents to the UERM Registrar’s Office (2/F Administration Building) during their enrollment schedule:

    1. Form 138 (Grade 12 Report Card)
    2. Certificate of Good Moral Character
    3. Certificate of Live Birth issued by the Philippine Statistics Authority
    4. Diploma or Certificate of Graduation
